Registered Nurse (NOCM)
COMMUNITY HEALTH NURSE
JOB PURPOSE:
The Community Health Nurse uses professional knowledge and discretionary judgment to provide assessment, health care planning and management and ongoing monitoring of individuals who have Developmental Disabilities and CCMC and to provide supervisory oversight to providers. This includes care for adults and children with complex medical conditions and others who additionally experience mental health diagnoses. The Community Health Nurse provides consultation, education and direction to individuals, families and staff.
QUALIFICATIONS:
- Current State of Alaska Registered Nurse License
- Must have 2 years of RN experience
- Must have Pediatric and Community Health experience
- Must have training in Nursing Oversight and Care Management
- Must be able to pass a Criminal Background Check
- Must have a valid Alaska driver’s license and at least 3 years of driving experience
- Must have a high level of interpersonal skills to handle sensitive and confidential situations.
- Position continually requires demonstrative poise, tact, and diplomacy
- Must be able to interact and communicate with individuals at all levels of the organization.
WORK ENVIRONMENT:
- Ability to work in an office setting
- Position may require extended hours including evenings and weekends, travel to clients residency.
- Prolong sitting
- May require travel to and from patient homes, school, community-based facility and or government institutions.
- Ability to lift 25 pounds
